Support patient admissions and care transitions for elderly populations, utilizing your sales and marketing expertise. As a Care Transition Coordinator, you’ll work primarily in the market, managing referrals and ensuring the facility is the provider of choice.
Key Responsibilities
- Generate, manage, and track inquiries or referrals.
- Implement effective marketing plans in collaboration with facility managers.
- Serve as liaison between facilities, admissions, and the referral community.
