Key Responsibilities
  • Administer medications and treatments as prescribed
  • Monitor and document residents’ health; report changes to supervising RN or physician
  • Assist with activities of daily living (ADLs) such as bathing, dressing, and feeding
  • Collaborate with rehabilitation and therapy teams
  • Ensure accurate, timely documentation compliant with policies and regulations
  • Offer emotional support and education to residents and families
  • Maintain a safe, clean environment using correct infection control procedures
Qualifications
  • Current and valid LPN license
  • Strong clinical skills, medication administration, and wound care experience
  • Excellent communication and teamwork skills
  • Patient-focused approach to nursing
  • BLS/CPR certification required
