Civic Center Health and Rehabilitation offers a supportive, home-like environment specializing in both long-term care and rehabilitation. This role enables nurses to build meaningful relationships with residents and participate actively in their care journeys.

Key Responsibilities
  • Administer prescribed medications and treatments
  • Monitor and document residents’ health and vital signs
  • Assist residents with activities of daily living (ADLs)
  • Collaborate with physical, occupational, and speech therapists
  • Maintain accurate and timely documentation
  • Provide emotional support to residents and families
  • Ensure infection control and a safe environment
Qualifications
  • Current and valid LPN license
  • Strong clinical skills, including medication administration and wound care
  • Excellent communication and teamwork abilities
  • Compassionate, patient-focused care approach
  • BLS/CPR certification
