About the job
Join a compassionate health and rehabilitation team providing long-term care and rehabilitation services to residents. As a Licensed Practical Nurse (LPN), you will deliver direct nursing care, support residents' recovery goals, and collaborate with an interdisciplinary healthcare team.
Responsibilities
- Administer prescribed medications and treatments.
- Monitor and document residents’ health status and report changes.
- Assist residents with activities of daily living (ADLs).
- Collaborate on rehabilitation and care plans with other professionals.
- Maintain documentation according to state and facility regulations.
- Provide support and education to residents and families.
- Uphold infection control and safety protocols.
Qualifications
- Current and valid LPN license.
- Strong clinical skills.
- Compassionate, patient-focused attitude.
- Good communication and teamwork skills.
- BLS/CPR certification required.
