Key Responsibilities

• Provide and oversee chronic care management for patients
• Develop and adjust care plans in collaboration with families and providers
• Monitor progress, facilitate education, and use telehealth for regular follow-ups
• Delegate tasks to care coordinators, maintaining high standards of care
• Maintain accurate health records and coordinate care with interdisciplinary teams
• Support medication reconciliation, scheduling, and community resource access

Qualifications

• Registered Nurse (RN) license (New Jersey or Compact)
• Experience with chronic and acute care
• Telehealth proficiency and EHR familiarity
• Strong leadership and communication skills
