Become a pivotal part of an interdisciplinary team focused on providing social work services to patients and their families during challenging healthcare journeys. This role centers on assessing and addressing psychosocial needs, offering support from admission through discharge, and helping patients and caregivers navigate complex end-of-life scenarios.
Key Responsibilities
- Conduct psychosocial assessments, brief therapy, and crisis interventions.
- Assist with comprehensive and bereavement risk assessments.
- Educate and counsel patients and families about illness adjustment, grief, and available resources.
- Participate in developing and updating the Plan of Care.
- Refer patients and families to community resources.
- Facilitate Advance Directives education and support.
- Serve as a liaison among patients, families, and outside agencies.
- Participate in discharge planning and placement alternatives.
- Support team collaboration and provide coverage during weekends as needed.
Qualifications
- Master's in Social Work from an accredited program.
- Active LMSW or LLMSW license in Michigan.
- Prefer 2 years’ experience (minimum 1 year in healthcare).
Preferred Knowledge and Skills
- Experience with terminal illness, loss, and grief support.
- Valid driver’s license and reliable vehicle.
- Up-to-date immunizations and TB testing.
