This role provides physical therapy services in an outpatient clinic setting in Phoenix, Arizona. The focus is on delivering evidence-based treatments to patients with various orthopedic, neurological, and musculoskeletal conditions within a collaborative, team-oriented environment.

Key Responsibilities

• Perform comprehensive physical therapy evaluations and functional assessments
• Develop and implement individualized treatment plans
• Employ manual therapy, therapeutic exercise, and modalities
• Reassess and adjust treatment based on progress
• Collaborate with referring providers and interdisciplinary team
• Educate patients and families on diagnoses and home care
• Maintain timely clinical documentation
• Promote preventative care and wellness
• Uphold professional and ethical standards

Qualifications

• Graduate of a CAPTE-accredited Physical Therapy program (Master's required, DPT preferred)
• Licensed or license-eligible Physical Therapist in Arizona
• Strong clinical documentation skills
• Excellent communication and organizational abilities
• Experience in a high-volume, team-based outpatient setting preferred
