As a Social Worker for the Kidney Complex Care Transplant Team in Phoenix, AZ, you will provide comprehensive psychosocial support to patients undergoing kidney transplantation. The role involves collaborating with an interdisciplinary team to address emotional, social, and financial needs throughout the transplant process.

Key Responsibilities

• Conduct psychosocial assessments and develop care plans
• Offer counseling and brief behavioral interventions
• Assist patients in accessing financial and community resources
• Educate patients and families about transplant expectations
• Maintain patient records and documentation

Qualifications

• Master’s degree in Social Work (MSW) required
• Arizona LMSW license required
• One year of medical or clinical experience preferred

Additional Qualifications

• Familiarity with chronic illness and transplant care
• Strong communication and coordination skills
• Certification in Transplant Social Work preferred or willingness to obtain
