Patient Care Manager

This Care Manager role focuses on coordinating and providing mental health care services. The position involves interdisciplinary collaboration to ensure quality, recovery-oriented support for individuals and families needing behavioral health assistance.

Key Responsibilities

• Coordinate care plans with various team members and community stakeholders
• Document treatment plans, crisis/safety plans, and various case management records
• Monitor client progress and respond to changing needs
• Provide case management, crisis de-escalation, life skills training, and support services
• Recognize and respond to high-risk situations
• Participate in quality improvement initiatives and maintain professionalism

Qualifications

• Bachelor's or associate's degree in a health or behavioral health field, or equivalent experience
• Valid driver's license; ability to travel as needed
• Knowledge of community health resources
• Must be able to obtain Arizona Fingerprint Clearance Card

Work Environment

• 100% office-based, located in Arizona
• May require occasional travel to tribal nations
