Key Responsibilities
- Coordinate care plans with various team members and community stakeholders
- Document treatment plans, crisis/safety plans, and various case management records
- Monitor client progress and respond to changing needs

- Recognize and respond to high-risk situations
- Participate in quality improvement initiatives and maintain professionalism
Qualifications
- Bachelor's or associate's degree in a health or behavioral health field, or equivalent experience
- Valid driver's license; ability to travel as needed
- Knowledge of community health resources
- Must be able to obtain Arizona Fingerprint Clearance Card
Work Environment
- 100% office-based, located in Arizona
- May require occasional travel to tribal nations
