Vitability Health is transforming the delivery of patient care through its Remote Patient Monitoring (RPM) program. The Patient Care Manager role is based in Mahwah, NJ, and is designed for professionals who thrive in a fast-paced, remote environment, managing Medicare patients and performing telehealth services.
Key Responsibilities
- Build trusting patient relationships to drive health behavior change using motivational techniques.
- Collaborate in care planning and provide education and coaching on chronic condition management.
- Monitor patient data and schedule audio-visual telehealth visits via dedicated software platforms.
- Document interactions, manage alerts, and escalate care to physicians when needed.
Qualifications
- MA/LPN/LVN with unrestricted New York license.
- Experience with Medicare and virtual patient management.
- Bilingual (Spanish) proficiency highly preferred.
- Self-motivated with strong communication skills and technical proficiency.
