Patient Care Manager

About the job

This position is responsible for managing a caseload of patients with chronic medical or mental health conditions, collaborating with providers and staff to ensure high-quality remote and in-person care. The Patient Care Manager plays a key role in care coordination, patient education, and fostering self-management for optimal health outcomes.

Key Responsibilities

• Manage assigned panel of chronic care patients.
• Collaborate with physicians/providers on care management.
• Provide follow-up to ensure compliance with individualized health plans.
• Offer telephone advice and coordinate urgent responses.
• Support patients in setting and achieving SMART self-management goals.
• Assess and address barriers to patient care plan adherence.
• Maintain thorough documentation and participate in quality improvement activities.
• May make home visits and coordinate care transitions.

Qualifications

• Bachelor’s degree in health care administration, informatics, or related field required.
• Licensed Practical Nurse (LPN) required; RN preferred.
• Experience with case management and chronic disease care preferred.
• Strong collaborative and communication skills.

Work Environment

• Based in Spring Valley, New York; may involve remote care delivery.
