About the job
This role involves supporting the delivery of quality, cost-effective healthcare outcomes for members through holistic care management activities in the Dallas, TX area and nearby counties. Interaction is required with both remote work and in-person member visits three days per week.
Key Responsibilities
- Assess members' needs and available resources
- Develop and monitor long-term care plans and service options
- Coordinate communication among members, caregivers, and providers
- Support members and families with referrals for employment, housing, and health services
- Educate members on healthcare procedures and benefit utilization
- Maintain compliance with relevant policies and documentation standards
Qualifications
- Bachelor's degree or equivalent experience required
- 2–4 years of related experience in care management
- Preferred: 3+ years case management with adult populations; experience in social work or case management in hospitals or community settings
