Develop and oversee care management activities for pediatric members with mental and behavioral health needs. This position involves creating care plans, facilitating service access, and coordinating with multidisciplinary teams to ensure high-quality, cost-effective outcomes in compliance with organizational and legal guidelines.
Key Responsibilities
- Assess member needs via phone or in-home visits
- Develop and facilitate ongoing personalized care/service plans
- Coordinate with families, providers, and community resources
- Monitor care plans and adjust as needed for optimal patient outcomes
- Complete and maintain required documentation
- Provide education related to mental health and substance use disorders
Qualifications
- Master’s degree in Social Work or related field
- At least 1 year of pediatric professional experience
- LCSW or LMHC required, must reside in the specified Florida counties
