Engage in care management activities that enable quality and cost-effective healthcare outcomes for members with long-term care needs. Work in a hybrid setting, meeting with members at homes or facilities, and collaborating with care providers, families, and community resources.
Key Responsibilities

- Coordinate between members, caregivers, and the care team

Qualifications
- Bachelor’s degree and 2–4 years of related experience (or equivalent)
