Patient Care Manager

About the job

Engage in care management activities that enable quality and cost-effective healthcare outcomes for members with long-term care needs. Work in a hybrid setting, meeting with members at homes or facilities, and collaborating with care providers, families, and community resources.

Key Responsibilities

• Assess member needs and resources to develop personalized service plans
• Coordinate between members, caregivers, and the care team
• Monitor care plans, update based on outcomes
• Facilitate education for members and families regarding procedures and benefits
• Maintain documentation compliant with regulatory requirements
• Provide feedback to leadership and support continuous improvement efforts

Qualifications

• Bachelor’s degree and 2–4 years of related experience (or equivalent)
