Support patient engagement and navigation at HealthPoint, the largest medical home for newly arrived refugees in King County, WA. Provide resources, care coordination, and facilitate wellness activities for diverse patient populations, focusing closely on refugee families.

Key Responsibilities

• Conduct outreach, engagement, and navigation support for patients
• Collaborate with care teams and community partners
• Help patients access services, schedule appointments, and connect with resources
• Respond flexibly to changing programmatic priorities and support organizational initiatives
• Manage patient scheduling, maintain records, and document encounters

Qualifications

• High School Diploma or GED required; 1+ years’ related experience
• Valid Washington State driver’s license
• Preferred: Associate degree, healthcare experience, and bilingual proficiency
