Provide social service support and counseling to patients and families in the Clear Lake area, focusing on assessment, treatment planning, and discharge coordination. Work collaboratively with clinical teams to ensure holistic care and support during and after agency service periods.

Key Responsibilities

• Deliver social services to patients and families, including assessment and planning
• Consult with staff to address patient and family needs
• Collaborate on discharge planning when care is no longer appropriate
• Provide written documentation and assessments for patient records
• Participate in relevant in-service programs and trainings

Qualifications

• Licensed Social Worker in Texas required
• Minimum one year of experience post-licensure
• Experience with hospice or home health is a plus
• Strong communication skills and clean background/driving record
