About the job
Deliver emotional and social support to patients and families in the Austin area as an integral part of their care teams. Responsible for assessments, planning, consultation, and ensuring smooth discharge transitions when care is no longer required.
Key Responsibilities

- Assist with discharge planning, including documentation and assessment

Qualifications
- Licensed Social Worker in Texas
- Minimum one year of post-licensure experience

- Clean background, drug screening, and driving record required
