This full-time social worker position is based in the Austin, Texas area, providing essential support to patients and their families. The role encompasses treatment, planning, and collaborating with other staff to ensure comprehensive care. Regular responsibilities also include assisting with discharge planning, maintaining records, and participating in relevant in-service programs.

Key Responsibilities

• Deliver social services as they relate to patient care
• Consult with staff to plan treatments and care
• Assess patients' and families' emotional and social needs
• Prepare written summaries for discharge planning
• Maintain patient records in accordance with requirements
• Participate in team and in-service meetings

Qualifications

• Licensed Social Worker in Texas (required)
• 1+ years' experience post-licensure (required)
• Hospice or Home Health experience (preferred)
• Effective written and verbal communication
• Clean background, drug screen, and driving record
