About the job
This position provides neurobehavioral health services to elderly residents in long-term care facilities, with a multidisciplinary approach. The therapist collaborates closely with care teams and families to deliver specialized support for neurocognitive and psychiatric conditions.
Key Responsibilities
- Conduct evaluations and provide condition-specific or insight-based therapy
- Coordinate care plans with facility staff, primary care providers, and families
- Act as part of an interdisciplinary treatment team
- Complete documentation in electronic medical records (EMR)
- Engage with families as needed for continuity of care
Qualifications
- Master's degree in Mental Health Counseling or Social Work
- Active state licensure as an LCSW or LMHC
- Strong time management and autonomous working capabilities
- Experience in geriatrics and/or inpatient or extended care preferred
- Familiarity with bedside assessment screens and EMR systems
