In this role, you will provide physical therapy services with a focus on home health care, collaborating with patients and caregivers to develop and implement personalized treatment plans in Early, Texas. You will be responsible for assessments, education, and reporting, while adhering to safety and regulatory guidelines.

Key Responsibilities

• Perform interdisciplinary assessment and develop individualized care plans
• Provide physical therapy treatments and patient education
• Prepare clinical notes and progress summaries
• Communicate with healthcare team and supervise assistants
• Uphold organization quality standards and participate in improvement programs

Qualifications

• Licensed Physical Therapist within the state
• Degree in physical therapy from CAPTE-accredited program
• Minimum two years' experience (home health preferred)
• Good communication and organizational skills
• Valid driver's license and insured vehicle
• CPR certification
