This position offers an opportunity to provide evidence-based assessment and psychotherapy to patients across the lifespan, within a flexible, hybrid environment split between home and clinic responsibilities. Key Responsibilities
- Conduct diagnostic assessments and provide individual, group, and family therapy
- Collaborate with patients in developing and monitoring treatment plans
- Coordinate care with primary care and specialty providers
- Deliver crisis support, triage, and risk assessment as needed
- Maintain thorough, patient-oriented clinical documentation
Qualifications
- Master’s degree in Psychology, Social Work, Counseling, or Nursing
- Valid Wisconsin clinical license (LCSW, LPC, LMFT)
- Certification by State of Wisconsin Medicaid/Health Forward program
- National Provider Identifier (NPI) required
- 3+ years direct clinical experience in behavioral health services
- Competency in evidence-based therapy and clinical documentation
- Commitment to inclusive, culturally competent care
