Key Responsibilities
- Evaluate members' needs and identify barriers to care.
- Develop personalized care and service plans in collaboration with providers and specialists.
- Identify problems and provide appropriate care management interventions.
- Coordinate care between members, families, caregivers, and healthcare teams.
- Monitor member status, update care plans as necessary, and ensure appropriate follow-up.
- Connect members and care managers with local resources (e.g., employment, housing, independent living support).
- Perform outreach via phone, digital, home, or on-site visits.
- Maintain thorough documentation to comply with regulatory requirements.
- Educate members and families on disease processes, care options, and referrals.
- Provide feedback on process improvements for enhanced care quality.
- Complete other duties as assigned to meet business needs.
Qualifications
- Degree from an accredited School of Nursing or a Bachelor's in Nursing.
- 2–4 years of related experience.
- Active RN licensure (state-specific and/or compact license).
