This position is dedicated to managing complex care needs, primarily focused on mental and behavioral health for members in Hillsborough, Polk, Manatee, Hardee, or Highlands County, Florida. The role requires local travel for member visits up to 50% of the time and remote work.

Key Responsibilities

• Conduct assessments via phone, home visits, and digital outreach
• Develop and monitor care plans for pediatric populations
• Coordinate services between families, providers, and community resources
• Provide education to families on mental health and substance use topics
• Maintain documentation for compliance and quality reporting

Qualifications

• Master’s degree in Social Work or related field
• LCSW or LMHC required
• Minimum one year of pediatric care experience
• Must reside in the specified Florida counties
