This field-based care management position focuses on holistic, person-centered care for long-term care members. The manager develops, implements, and monitors care plans, coordinating resources and conducting field visits for assessments in the Citrus County area of Florida.

Key Responsibilities

• Assess member needs and resources for care plan development
• Conduct field visits and assessments for long-term care members
• Coordinate care between providers, members, and families
• Provide resource support for employment, housing, and independent living
• Educate members and caregivers on available benefits
• Maintain documentation in compliance with regulations
• Monitor and adjust care plans based on outcomes and needs

Qualifications

• Bachelor’s degree and 2–4 years of related experience
• Strong case management background preferred
• Must reside in designated Citrus County zip codes
