Be an integral part of patient-centered preventative care as a Patient Care Manager, overseeing a panel of chronic care patients and collaborating with multidisciplinary teams. This position is designed to improve patients’ health outcomes through coordination, self-management promotion, and proactive intervention.
Key Responsibilities
- Manage a caseload of chronic care and mental health patients

Qualifications
- Bachelor’s degree in healthcare administration, informatics, or related field
- Licensure as a Licensed Practical Nurse (LPN) or Registered Nurse (RN) preferred

- Experience with mental health preferred
