About the job
As a Patient Care Manager in New York, you will lead the assessment, planning, and coordination of complex care for members, ensuring quality and cost-effective healthcare outcomes. Collaborate with care teams to develop personalized care plans, monitor clinical progress, and support both new and existing staff in achieving optimal patient care.
Key Responsibilities

Qualifications
