Care managers support remote chronic disease and behavioral health care for patients in their homes or assisted living, ensuring seamless care coordination and positive health outcomes.
Role Overview
Work in a mission-driven environment providing Chronic Care Management and Behavioral Health Integration, focusing on patient empowerment, health monitoring, and care plan implementation.
Key Responsibilities
- Collaborate with providers and practice staff to identify patients needing care management
- Develop and manage individualized care plans, including mental health concerns
- Educate patients on self-management tasks and monitor progress
- Coordinate with health teams for hospital discharges, referrals, and treatment plan adjustments
- Manage documentation and compliance according to healthcare standards
- Respond to urgent patient concerns and maintain continuous communication
Qualifications
- 3+ years’ experience as a Care Manager, CNA, or Licensed Practical Nurse
- Knowledge of Patient-Centered Medical Home model
- Experience managing chronic diseases and preventive care
- Valid driver’s license for occasional home visits
- Proficiency in English, bilingual a plus

Work Hours
