About the job
Take on a leadership role managing and coordinating patient care within a home health or hospice setting. Oversee clinical personnel, ensure quality of care, and support interdisciplinary collaboration to meet patient and family needs.
Key Responsibilities
- Coordinate patient care and assign appropriate clinical staff

Qualifications
- Graduate of NLN-accredited nursing school, BSN preferred
- Registered nurse with current state license
- Experience in home care and two years of management or supervisory experience preferred
- Excellent communication, observation, and judgment skills
- Valid driver's license and current CPR certification required
