Physical Therapist

This position provides essential physical therapy services to patients in home health settings, fostering recovery and improved quality of life. The role focuses on comprehensive assessment, development of individualized care plans, and collaboration with patients, families, and healthcare professionals.

Key Responsibilities

• Conduct interdisciplinary assessments and partner with patients and caregivers to develop care plans
• Deliver education and counseling to patients and families
• Prepare clinical notes and progress summaries on treatment goals
• Communicate directly with physicians regarding patient status and updates
• Supervise Physical Therapy Assistants and home health aides as required by policy

Qualifications

• Licensed as a physical therapist within the state of practice
• Degree from a CAPTE-accredited program
• Two years of relevant experience, preferably in community/home health
• Current CPR certification and valid driver’s license
• Ability to travel as required for home visits
