Key Responsibilities
- Conduct interdisciplinary assessments and partner with patients and caregivers to develop care plans

- Communicate directly with physicians regarding patient status and updates
- Supervise Physical Therapy Assistants and home health aides as required by policy
Qualifications
- Licensed as a physical therapist within the state of practice
- Degree from a CAPTE-accredited program
- Two years of relevant experience, preferably in community/home health
- Current CPR certification and valid driver’s license
- Ability to travel as required for home visits
