The medical social worker helps implement and uphold standards of care for medical social work, focusing on psychosocial assessment, evaluations, and intervention planning. This part-time position involves maintaining clinical records, engaging with care teams, and participating in interdisciplinary meetings.

Key Responsibilities

• Assess psychosocial status of terminally ill patients and families.
• Provide evaluations, counseling, and practical support.
• Maintain collaborative relationships with care teams and community agencies.
• Document and maintain accurate records of services.
• Participate in the development of care plans and quality improvement teams.

Qualifications

• Bachelor's degree in social work from an accredited program.
• Meets state and federal personnel requirements.
• At least 1 year of healthcare or hospice experience preferred.
• Current CPR certification.
• Licensed driver with appropriate automobile insurance.
