As a Social Worker-Counselor-Licensed, you will provide case management, psychosocial support, and coordinate patient discharges, working under the direction of Clinical Social Work Services.

Key Responsibilities

• Manage applications of clinical diagnostics, assessments, and treatment in a healthcare setting.
• Identify and address psychosocial factors affecting patients' care.
• Actively participate in discharge planning for patient transitions.
• Work with multidisciplinary teams to optimize healthcare utilization.

Qualifications

• Master’s degree in Social Work required.
• Active Massachusetts LCSW license required.
• Three years of related work experience, with at least one in a medical setting.
• Computer proficiency necessary.
• Certification in Case Management preferred.
• Compliance with health vaccination policies.

Location

On-site at St. Luke's Hospital, New Bedford, Massachusetts, with day shift hours and occasional holiday rotations.
