As a Social Worker-Counselor-Licensed, you will provide case management, psychosocial support, and coordinate patient discharges, working under the direction of Clinical Social Work Services.
Key Responsibilities
- Manage applications of clinical diagnostics, assessments, and treatment in a healthcare setting.
- Identify and address psychosocial factors affecting patients' care.
- Actively participate in discharge planning for patient transitions.
- Work with multidisciplinary teams to optimize healthcare utilization.
Qualifications
- Master’s degree in Social Work required.
- Active Massachusetts LCSW license required.
- Three years of related work experience, with at least one in a medical setting.
- Computer proficiency necessary.
- Certification in Case Management preferred.
- Compliance with health vaccination policies.
