Key Responsibilities

• Oversee clinical care delivery coordination within the branch.
• Facilitate communication among care teams, patients, families, and external partners.
• Mentor and train field staff on rules, regulations, and best clinical practices.
• Promote accountability and teamwork within the branch environment.
• Utilize tools to optimize clinical coding, service usage, and achieve patient-specific goals.

Qualifications

• Registered Nurse (RN) License valid in the state of practice.
• At least two years of RN experience in clinical settings; one year in home health preferred.
• Previous management experience is a plus.

Required Skills

• Effective teamwork and leadership abilities.
• Strong communication (written and oral).
• Ability to manage multiple priorities and remain accountable to goals.
