Key Responsibilities

• Evaluate and manage care for complex, high-risk members with mental/behavioral health needs.
• Act as a liaison and advocate between members, caregivers, physicians, and agencies.
• Conduct home or site visits to assess member needs and provide crisis support.
• Educate members and families on preventive care, treatment options, and benefits.
• Coordinate community resources and facilitate service authorizations.
• Ensure compliance with clinical and regulatory guidelines.

Qualifications

• Master's degree in Mental Health or Social Work, or RN degree.
• 2-4 years of related experience.
• Licensed Behavioral Health Professional or RN as per state requirements.

Compensation and Benefits

• Pay Range: $26.50 - $47.59 per hour.
