Key Responsibilities
- Evaluate and manage care for complex, high-risk members with mental/behavioral health needs.
- Act as a liaison and advocate between members, caregivers, physicians, and agencies.
- Conduct home or site visits to assess member needs and provide crisis support.
- Educate members and families on preventive care, treatment options, and benefits.
- Coordinate community resources and facilitate service authorizations.
- Ensure compliance with clinical and regulatory guidelines.
Qualifications
- Master's degree in Mental Health or Social Work, or RN degree.
- 2-4 years of related experience.
- Licensed Behavioral Health Professional or RN as per state requirements.
Compensation and Benefits
- Pay Range: $26.50 - $47.59 per hour.
