Take on a crucial nursing leadership role managing complex care activities in New York for a diverse member population. This role involves assessing, planning, and implementing high-quality case management to ensure optimal outcomes for members, in compliance with regulatory standards. You'll support both care delivery and clinical staff onboarding as part of a collaborative team.

Key Responsibilities

• Develop and assess ongoing personalized care plans for members
• Coordinate care among members, families, and healthcare teams
• Monitor progress and update care plans as needs evolve
• Train and mentor new clinical hires, providing coaching and feedback
• Ensure regulatory compliance and maintain thorough documentation
• Facilitate education for members and families on healthcare options and benefits

Qualifications

• Degree from an accredited School of Nursing or Bachelor's in Nursing
• 4–6 years of related experience
• Active New York State RN license (or Compact State Licensure)
