Join a transformative healthcare team dedicated to delivering holistic care management for members with significant mental and behavioral health needs in various regions of Texas. This role focuses on assessing, coordinating, and supporting quality, cost-effective healthcare outcomes for members experiencing complex behavioral health challenges.
Key Responsibilities
- Evaluate complex, high-risk members with behavioral health needs to recommend the best care plans.
- Act as a liaison and advocate between members, families, and service providers.
- Support members with a range of mental health needs, including major depression, bipolar disorders, schizophrenia, and substance use disorders.
- Conduct frequent home or site visits to assess member needs and collaborate with local resources.
- Educate members and caregivers regarding care options, preventive care, treatment plans, and benefits.
- Coordinate services across medical, behavioral, and social domains, ensuring accessibility and seamless referrals.
- Maintain thorough and accurate documentation within electronic systems and adhere to compliance standards.
- Participate in improving the quality and effectiveness of care programs.
Qualifications
- Master's degree in Mental Health, Social Work, or graduate of an Accredited School of Nursing.
- Possession of LCSW, LPC, or RN with behavioral health experience (required).
- 2–4 years of related experience in case management, care coordination, or mental health services.
- Experience in field-based or inpatient settings preferred.
