This role focuses on assessing, coordinating, and supporting quality, cost-effective healthcare outcomes for members experiencing complex behavioral health challenges.

Key Responsibilities

• Evaluate complex, high-risk members with behavioral health needs to recommend the best care plans.
• Act as a liaison and advocate between members, families, and service providers.
• Support members with a range of mental health needs, including major depression, bipolar disorders, schizophrenia, and substance use disorders.
• Conduct frequent home or site visits to assess member needs and collaborate with local resources.
• Educate members and caregivers regarding care options, preventive care, treatment plans, and benefits.
• Coordinate services across medical, behavioral, and social domains, ensuring accessibility and seamless referrals.
• Maintain thorough and accurate documentation within electronic systems and adhere to compliance standards.
• Participate in improving the quality and effectiveness of care programs.

Qualifications

• Master's degree in Mental Health, Social Work, or graduate of an Accredited School of Nursing.
• Possession of LCSW, LPC, or RN with behavioral health experience (required).
• 2–4 years of related experience in case management, care coordination, or mental health services.
• Experience in field-based or inpatient settings preferred.

Work Hours

• Monday to Friday, 8 am – 5 pm (CST)
