This managerial care management role works with high-acuity, complex medical populations to coordinate all aspects of long-term care. The position is based in Texas and requires field visits, close collaboration with clinical teams, and the development and ongoing assessment of personalized service plans.
Key Responsibilities
- Assess and coordinate care for complex and high-risk members
- Develop, implement, and monitor long-term care plans
- Collaborate with interdisciplinary teams and service providers
- Conduct face-to-face visits in member homes as needed
- Maintain compliance with clinical guidelines and regulations
- Educate members and families about health processes and resources
- Mentor and precept new clinical hires
Qualifications
- Graduate from accredited nursing program or Bachelor’s degree
- 4–6 years related experience, RN license required
- 2 years pediatric experience preferred
- Strong organizational and communication skills
Compensation & Benefits
- Pay range: $35.49–$63.79 per hour
