Patient Care Manager

About the job

This managerial care management role works with high-acuity, complex medical populations to coordinate all aspects of long-term care. The position is based in Texas and requires field visits, close collaboration with clinical teams, and the development and ongoing assessment of personalized service plans.

Key Responsibilities

• Assess and coordinate care for complex and high-risk members
• Develop, implement, and monitor long-term care plans
• Collaborate with interdisciplinary teams and service providers
• Conduct face-to-face visits in member homes as needed
• Maintain compliance with clinical guidelines and regulations
• Educate members and families about health processes and resources
• Mentor and precept new clinical hires

Qualifications

• Graduate from accredited nursing program or Bachelor’s degree
• 4–6 years related experience, RN license required
• 2 years pediatric experience preferred
• Strong organizational and communication skills

Compensation & Benefits

• Pay range: $35.49–$63.79 per hour
