In this remote position, you will play a pivotal role in developing, assessing, and facilitating complex care management activities for members with primary physical health needs in Kansas. Your efforts will ensure the delivery of high-quality, cost-effective healthcare, driven by personalized care plans and comprehensive member education.
Key Responsibilities
- Evaluate member needs and barriers to care, including identifying social determinants of health
- Develop ongoing care and service plans, collaborating with healthcare providers and community resources
- Identify and address problems or barriers to coordinate effective care management interventions
- Facilitate communication among members, caregivers, and care provider teams to ensure person-centered care
- Provide regular follow-ups and monitor member status, adjusting care plans as needed
- Maintain thorough documentation to comply with regulatory standards
- Educate members and families on disease processes, care options, and healthcare benefits
Qualifications
- Degree from an accredited School of Nursing or a Bachelor's in Nursing
- 2–4 years of related experience required
- Valid RN license or Compact State Licensure
