Be an essential part of patient rehabilitation at a skilled nursing facility, providing evidence-based physical therapy care following physician referral. This role emphasizes patient-centered support, collaboration, and ongoing professional growth in a post-acute care environment.
Key Responsibilities
- Deliver direct physical therapy treatment and monitor patient progress
- Set and adjust functional, measurable goals tailored to patient needs
- Supervise PTAs, Rehab Aides, and students
- Maintain timely and accurate documentation using therapy software
- Support interdisciplinary patient care planning and strong communication
Qualifications
- Graduate of an accredited BSPT, MSPT, or DPT program
- Licensed in physical therapy within the state
- CPR certification required (upon hire or during orientation)
- Experience in a post-acute or related setting is preferred
