Physical Therapist

About the job

Provide home-based physical therapy services focusing on patient-centered care and recovery. This position ensures individualized assessment, care plan development, and delivery of therapy under established protocols within a supportive, interdisciplinary team.

Key Responsibilities

• Assess and develop individualized care plans in collaboration with patients and caregivers
• Deliver patient, caregiver, and family counseling and education
• Prepare clinical notes, progress summaries, and document outcomes
• Communicate with physicians and healthcare team on care plans
• Conduct therapy, evaluate home environment, and direct treatments
• Supervise physical therapy assistants and participate in case conferences
• Participate in organization’s quality assessment and training programs

Qualifications

• Licensed to practice as a physical therapist in the state
• Degree from an accredited CAPTE program
• At least two years of experience preferred (home health experience preferred)
• Good communication and organizational skills
• Current CPR certification required
• Valid driver's license, insured vehicle
