The Legacy Living and Rehabilitation Center in Gillette, Wyoming, is dedicated to providing respectful and dignified long-term and short-term rehabilitation services. As a social worker, you'll play a key role on the care management team, supporting residents' mental and psychosocial well-being while ensuring seamless quality care throughout their stay.

Key Responsibilities

• Participate in interdisciplinary resident assessment and care planning
• Lead and coordinate overall plan of care meetings
• Facilitate communication and referrals for residents and families
• Ensure compliance with CMS standards and facility policies
• Support grievance process and contribute to quality improvement
• Maintain professional relationships with staff, service providers, and families
• Deliver continuing education and contribute to accreditation readiness

Qualifications

• Bachelor's degree in Social Work or a related human service field
• Minimum 1 year of social services experience
• Ability to collaborate within a team and advocate for residents
• Certifications as required by facility policies
