Patient Care Manager

This Patient Care Manager position is focused on developing and facilitating care management for members with mental and behavioral health needs in Texas. Candidates must possess the required licensure (LCSW, LMFT, LPC, or RN with behavioral health experience) and reside in Texas for in-person meetings.

Key Responsibilities

• Assess member needs through phone and in-home visits for mental health, substance use, and social determinants.
• Develop and update individualized care and service plans for members with elevated acuity.
• Collaborate with healthcare providers and community resources.
• Monitor and adjust care plans based on member progress and evolving needs.
• Document all care management activities to ensure compliance.
• Provide education to members and families about resources, procedures, and benefits.
• Support program improvements and cost-effective care delivery.

Qualifications

• Master’s degree in Behavioral Health or Social Work, or RN degree with 2-4 years of related experience.
• Licensed Behavioral Health Professional: LCSW, LMFT, LPC, or RN (Texas).
• Experience in behavioral health, assessment, and care coordination.
