Patient Care Manager

About the job

The Care Manager leads the management of care for Medicaid youth and adults with complex behavioral health and foster care needs. The position requires developing and facilitating personalized care plans, collaborating with families, resources, and providers, and providing education on mental health and substance use disorders.

Key Responsibilities

• Assess patient needs via phone or in-home visits
• Develop and maintain individualized care plans for high-acuity cases
• Coordinate services among providers, specialists, and community resources
• Monitor and adjust plans based on member status
• Maintain rigorous documentation per compliance standards
• Educate members and families about treatment options and care benefits

Qualifications

• Master's degree in Behavioral Health, Social Work, or Nursing
• 2–4 years of related experience
• State-appropriate licensure (e.g., LCSW, LMFT, RN)
• Background in foster care and Medicaid preferred
